Cialis (Tadalafil) #

Cialis is related to a group of phosphodiesterase type 5 (PDE5) inhibitors, and believed to be one of the most effective medications for treatment of erectile dysfunction (ED).

ED drug therapy with PDE-5 inhibitors is currently considered as the most effective treatment for male sexual dysfunction.

Indications

  • In 2003 – Cialis was approved by the FDA as an effective drug for erectile dysfunction treatment in adult males.
  • In 2011 – FDA approved Cialis for symptomatic treatment of benign prostate enlargement (BPE), or benign prostatic hyperplasia (BPH).
  • In 2016 – Cialis pills are prescribed to treat erectile dysfunction, BPE, or BPE-associated ED.

Cialis is intended to treat erectile dysfunction of any severity and etiology (physiological or psychological). For BPE treatment, Cialis can be administered in combination with anticancer hormonal drug Finasteride. The duration of the combination therapy of BPE and ED should not exceed 26 weeks.

Mechanism of action

The active ingredient in Cialis is Tadalafil. This ingredient is characterized by a pharmacological property to inhibit action of phosphodiesterase 5 enzymes, and thereby to relax the penile smooth muscles.

Due to Tadalafil action, the blood flow to the penis is increased, the cavernous tissues are filled with blood, resulting in an erection sufficient for sexual intercourse.

Active ingredient Tadalafil is not meant to increase sexual desire. Therefore, Tadalafil-containing pills Cialis should not be used “off-label” to enhance libido.

Pharmacokinetics

Cialis is rapidly absorbed in the gastrointestinal tract. The maximum plasma concentration of the active substance is achieved at the average within 2 hours after a single oral-dose Cialis.

Cialis half-life makes 17.5 hours. Fatty foods or alcohol (small amount) does not affect Cialis bioavailability. Pills for ED treatment Cialis are metabolized in the liver and excreted primarily in feces, to a lesser extent with urine.

Dosage forms and strengths

  • Cialis is available in tablet form. Yellow, almond-shaped Cialis pills may contain:
  • 20 mg of Tadalafil (“C 20” marking)
  • 10 mg of Tadalafil (“C 10” marking)
  • 5 mg of Tadalafil (“C 5” marking)
  • 5 mg of Tadalafil (“C 2 1/2” marking)

Dosage

Cialis drug is taken once a day, when it’s convenient, or at least half an hour before expected sexual activity. A pill should be taken whole, without splitting or crumbling. ED therapy can be carried out in two ways:

  1. The daily use of Cialis.
  2. The use of Cialis on an as needed basis.

An initial daily dose of Cialis for ED treatment is 2.5mg. If you want to enhance the effect of Cialis, the daily dose can be increased to 5mg. If for erectile dysfunction treatment, Cialis daily dose schedule is preferred, a pill of 2.5mg or 5mg is recommended to be taken at the same time.

If Cialis (Tadalafil) is used right before planned sexual activity (as needed basis), the recommended initial dose is 10 mg. Depending on clinical response obtained and tolerance, a Cialis dose can be increased to 20mg or lowered to 5mg per day.

The recommended Cialis dose for BPH treatment makes 5 mg per day. If combined BPH treatment includes Finasteride pills, a daily Cialis dose should not exceed 5mg.

Adjustment of daily doses of Cialis

In older men (65 years and above), men with normal liver and kidneys function, and men with mild renal insufficiency, Cialis dosage regimen should not be adjusted.

A recommended initial dose of Cialis for ED and BPH treatment in men with moderate renal insufficiency makes 5mg per day. The maximum dose of Cialis for this group of men makes 10mg every 48 hours.

Men with severe renal insufficiency are not recommended to take more than one 5mg pill of Cialis, once a day. Men with hepatic impairment are recommended to take minimum effective doses of Cialis.

Contraindications

Severe hepatic impairment and hypersensitivity to the active or additive ingredients of the drug are considered to be a clear contraindication to Cialis.

Sexual potency regulator Cialis is contraindicated for ED and BPH treatment in children and adolescents younger than 18 years. Simultaneous administration of Cialis with any medications containing organic nitrates is prohibited. Clinical studies have shown that Cialis enhances the hypotensive effect of nitrates and may cause serious side effects.

Adverse reactions

Usually Cialis (Tadalafil) is well tolerated in patients. Adverse effects are reported to be rare and mild. Normally, any discomfort and side effects pass away in further drug application.

The most frequent adverse reactions include headache, indigestion, flushing, and nasal congestion. Also ache in the back and muscles can be observed. In rare cases, spontaneous penile erection is reported.
